This resource section of our site will list various resources and links that will assist our clients in the medical industry. There is a growing need to work with patients that do not speak English. This need was discussed in detail in our blog post on the potentially dangerous situation in pharmacies today.

Our first resource is a free medical history document. This PDF is set up as a form and can be used in conjunction with the patient intake procedure. The links below provide the English version and the Spanish version for your reference.
